Return to Casper Hockey after injury and USAH's Concussion Management Protocol
Did you know that CAHC needs a signed medical release prior to your player returning to hockey after injury?
Regardless of where the injury occurred (family vacation, school, etc.), USAH requires a signed medical release be received by the local association prior to the player returning to hockey.
We are never happy when one of our Oilers gets hurt. But, we are very happy to have return to the ice! But, first we need that signed medical release. So, drop that off in the CAHC box at the rink and email Diane at firstname.lastname@example.org to let her know it is ready for pick up as she needs to forward that onto the WAHA Risk Manager.
If your player was injured at hockey, we need to know that too! Please email Diane at email@example.com so she can email you the USAH insurance forms that need to be completed!
USAH Concussion Protocol
USAH requires the following concussion management protocol to be followed and the form below needs to be completed prior to returning to hockey!
Please review the attachments below, as well as the numerous other resources available on the USAH website.
